Why “holistic” care?

Because you’re more than a symptom. At hhc+, we consider your biology, lifestyle, environment, nervous system, and history to uncover root causes and guide you toward meaningful, lasting healing.


Will I be seen by a doctor?

Yes. Your care team includes:

Dr. Meg Tantillo, DNP, FNP-BC is a Doctor of Nursing Practice and board-certified Family Nurse Practitioner licensed to evaluate, diagnose, prescribe and manage care in CT, MA, RI, NY, FL, CA and VA.

Dr. Patel, ND is a Naturopathic Doctor specializing in integrative and functional medicine.
He provides naturopathic and functional care for patients. He is licensed to practice naturopathic medicine in CT and VT. 

Dr. Brenda Gilmore, CNM, FNP is a Doctor of Nursing Practice, and board-certified Nurse Midwife and Family Nurse Practitioner with advanced training in women’s health and integrative care. She is licensed to evaluate, diagnose, prescribe, and manage care in CT. 

Dr. Claudia, ND, MScN, IFMCP, Is a naturopathic doctor with specialized training in IBD, fecal microbiota transplantation (FMT), and functional medicine. Her clinical focus includes gastrointestinal disorders, women’s health, and cardio-metabolic health. She is licensed in CT, MD and CA.

Elizabeth Mitchell, FNP-BC, IFMCP is a board-certified Family Nurse Practitioner licensed in multiple states, providing thoughtful, individualized care to both children and adults. She is licensed in NH, ME, VT, MA, CT, NY, RI, GA, FL and CA.

Can we meet in person?

Yes. We offer in-person care at our Farmington, CT clinic and virtual visits through our secure patient portal.


Do you replace my primary care provider?

No. We provide functional and holistic healthcare and do not replace your primary care provider or offer urgent or emergent services.


Do you see children?

Yes. Several hhc+ providers offer pediatric functional medicine for digestive issues, recurrent infections, behavioral concerns, skin issues and more.


Do you work with patients outside Connecticut?

Yes.

We offer care to patients both in and outside of Connecticut. Patients who reside in states where our providers are licensed are eligible for full medical care, including prescriptions, diagnostic testing, and ongoing treatment.

Patients located in other states may still work with our team through functional health consultations, which can include lifestyle, nutrition, and supplement guidance, based on provider licensure and state-specific scope of practice.

What will my visit be like?

Your first visit is a 75-minute comprehensive intake. Your provider reviews your history, symptoms, lifestyle, and goals, then develops a personalized care plan. You receive your written plan within 48 hours.

Follow-up visits review labs, track progress, and adjust your plan based on how your body is responding.


What are health coaching visits like?

Health coaching helps you implement your care plan and stay consistent. Sessions may include:

  • Nutrition and meal structure

  • Sleep optimization

  • Stress/nervous system support

  • Habit-building and accountability

  • Identifying barriers and creating action steps

Health coaching is not medical care—it is practical support to help you succeed.


How do I communicate with my provider?

Members have unlimited messaging through the portal for routine questions related to your established treatment plan. Messages are answered within 2 business days.

Urgent concerns should be directed to your PCP or emergency services.


Can my provider prescribe medications and supplements?

Yes. We work with patients across the United States. Medical services such as prescribing medications, ordering laboratory testing, and diagnosing conditions are provided in accordance with each provider's state licensure. When those services aren't available in your state, we can still provide education, lifestyle and nutrition recommendations, and personalized supplement guidance within the scope of applicable regulations.


Where do I complete the lab work?

Lab partners include:

  • Quest Diagnostics

  • Vibrant America

  • Rupa Health (DUTCH, GI Map, OAT, SIBO, mold panels, micronutrients, and more)

Many tests are completed at home. Rupa can also coordinate mobile phlebotomy and provide superbills.


What if I need diagnostic imaging?

If diagnostic imaging or a specialist referral is needed, we'll discuss the best next steps based on your location and your provider's licensure. When we're able to order imaging or place referrals directly, we'll coordinate that for you. If not, we'll work with your primary care provider or another local clinician to help ensure you receive the recommended testing or care.


How do I fill my supplement prescription?

We use Fullscript, which ships supplements directly to your home. Members receive 15% off all orders.


Do you offer IV therapy?

Yes. We offer signature and next-level IV drips for immune support, detoxification, energy, performance, and more. Members receive 15% off all drips and injections.


Do you offer hormone replacement therapy (BHRT)?

Yes. We provide comprehensive BHRT protocols with personalized dosing and ongoing monitoring.


Do you offer fertility support?

Yes. We offer functional fertility care including cycle mapping, ovarian optimization, hormone testing, thyroid support, metabolic workup, and preconception planning.


Do you treat complex or chronic conditions?

Yes—complex cases are our specialty. We commonly help patients with:

  • Autoimmune conditions

  • Thyroid disorders

  • Mast cell activation syndrome (MCAS) and histamine intolerance

  • Chronic infections, including Lyme disease, Bartonella, and Epstein-Barr virus (EBV)

  • Mold and environmental illness

  • Hormone conditions, including PCOS, PMS/PMDD, endometriosis, and perimenopause

  • Digestive concerns, including SIBO, IBS, dysbiosis, reflux, and food sensitivities

  • Fatigue, burnout, brain fog, and chronic inflammation

  • Anxiety, mood changes, and sleep concerns

  • Weight resistance and metabolic health

How much does the hhc+ membership cost? 

Membership is $1,590 for six months (paid upfront) or $275/month for six months.


Can I use my insurance benefits?

We do not participate with insurance or Medicare. If you have out-of-network benefits, you may request a superbill for possible reimbursement once the membership fee is paid in full.


What’s included in the membership?

Your membership includes six months of personalized, holistic care, including:

  • One 75-minute initial consultation with your provider

  • Your personalized care plan

  • Three 45-minute follow-up visits with your provider

  • Two 30-minute visits with your health coach

  • Unlimited messaging with your care team for routine questions related to your established treatment plan

  • Prior authorizations for medications, imaging, or laboratory testing ordered by your provider

  • Referrals and laboratory orders discussed during your scheduled visit

  • Routine medication refill requests (when follow-up recommendations have been met)

  • 15% off supplements, drips, and injections


Can I pay per visit?

After completing your initial six-month membership, you have the option to transition to per-visit care:

  • Follow-ups: starting at $310

  • Messaging support: $7/min

  • Chart review or Rx requests outside visits:$35


Do you offer payment plans?

Yes. Membership may be paid monthly at $275/month.


Do you offer refunds?

Membership fees are non-refundable, as our team begins preparing for your case immediately upon enrollment.


What happens after my six-month membership ends?

You may continue care by renewing your membership or switching to per-visit care.
Your provider will guide you based on your ongoing goals.


Can I purchase labs without becoming a patient?

No. Functional labs may only be ordered for active patients to ensure proper interpretation and safe integration into your care plan.


Can I receive a prescription before my first appointment?

No. Prescriptions require a full clinical intake.
