Use of this website does not establish a provider-patient relationship unless and until care is formally initiated through the clinic, including completion of required intake forms and acceptance into care.

3. Informational Use Only

Memberships and Clinical Services

Membership fees and payments for clinical services (including visits, programs, and care coordination) are non-refundable and non-transferable, unless otherwise explicitly stated in writing.

Memberships provide access to care, provider time, and clinical support over a defined period. Unused services or time do not roll over or convert to refunds.

Additional policies, including scheduling, cancellations, and communication expectations, are outlined in your Client Service Agreement and intake documents.

Retail Products (Limited Shop Items)

hhc+ may offer limited retail items such as apparel or branded merchandise.

For these items only:

  • Returns may be accepted within 14 days of delivery

  • Items must be unused and in original condition

  • Return shipping is the responsibility of the customer

  • Original shipping fees are non-refundable

7. Communication and Patient Messaging

For established patients, clinical questions, medication requests, and lab-related communication must be submitted through the secure patient portal.

The patient portal is the primary and most appropriate channel for clinical communication. Website forms, email, or SMS do not replace the portal and may not be appropriate for medical concerns.
