Functional Medicine NP


Location: Farmington, CT (In-Person, Hybrid, or Remote Considered)
Structure: Independent Contractor (1099)
Schedule: Flexible (Minimum 2 clinical days/week)

Holistic Health Code (hhc+) is a well-established functional and integrative medicine clinic with a strong reputation, consistent patient demand, and a highly engaged community.

This role is designed for a Nurse Practitioner who wants to practice real functional medicine with autonomy, depth, and the ability to build meaningful patient relationships.

You’ll be stepping into an environment where:

What Makes This Different

This is not a traditional NP role. You are not expected to:

  • Manage your own marketing

Instead, you’ll be supported by:

  • A steady flow of new patient inquiries

  • Full administrative and scheduling support

  • A clinical team and nursing infrastructure

  • Established systems for onboarding, follow-up, and patient management

You focus on clinical care and patient outcomes, and we handle the rest.

Clinical Responsibilities

  • Conduct comprehensive new patient visits and follow-ups

  • Develop personalized, root-cause-based care plans

  • Order and interpret advanced functional labs

  • Manage medications, supplements, and lifestyle interventions

  • Support patients across metabolic, hormonal, and complex chronic conditions

  • Maintain clear, timely documentation within the EHR

  • Collaborate with the team as needed to support patient outcomes

Compensation

  • 1099 Independent Contractor

Requirements

  • Active Connecticut APRN license

  • Full Practice Authority (FPA)

  • Active DEA license

  • Minimum 2 years NP experience

  • Experience or training in functional/integrative medicine

  • Strong clinical reasoning and ability to manage patients independently
