Membership is $1,590.

Alternatively, you can opt into a monthly payment plan of $275 monthly for 6 payments.

Our providers are out of network with all insurance plans including Medicare.

Members with out-of-network benefits in CT, MA, NY, FL, CA and ID may request an invoice detailing the service received and the ICD-10 and CPT codes needed by your insurance company to process your claim for reimbursement after the membership fee is paid in full.

Members are encouraged to work with their insurance plan to learn more about their specific claims process.

We accept FSA and HSA.

Yes, our providers are licensed to prescribe medication in CT, MA, NY, FL, CA, NV, MD, and ID. Our providers can recommend supplements nationwide.

Many test kits are completed at home. Blood work is typically ordered through Quest Diagnostics, Vibrant America or Rupa Health. Our team will coordinate your specific testing plan.

Membership fees are non-refundable and non-transferable. 

Membership requires a six-month commitment and does not automatically renew.

Should your needs change during your term, please reach out to hello@holistichealthcode.com so we can best support you through the remainder of your membership.

At the end of your membership, you may choose to renew for continued access to our high-touch, holistic care. If you do not renew, follow-up visits are available at $310 per appointment.
